Frequently Asked Questions

  • First, you may schedule a free consultation via our website. The therapist you choose to meet with may or may not have current availability to work with you, or may recommend that you connect with a different therapist on our team after the initial consultation. You are also welcome to request a certain therapist(s) or indicate that you have certain preferences for the kind of therapist you are hoping to work with. Completing an initial consultation with any therapist on our team is the best way to move forward with finding the right fit for you!

  • A therapeutic hour (53 minutes) with a student intern is $110 per session. This includes individual, couples and family sessions. Associate clinicians set their own rates.

    Group therapy sessions with student interns are $45 per hour. Associate clinicians set their own rates.

    Per request due to extenuating financial circumstances, a sliding scale may be offered. Forage Wellness Collective does not turn clients away due to their inability to pay, and is committed to providing appropriate mental health services to all.

  • Student interns at Forage Wellness have passed a competitive interview process and are dedicated to excellence as Restoration Therapists. They work closely with their clinical supervisor to ensure quality of care, and they work with many insurance plans as well!

  • Yes! All of our therapists work with Cigna, CareOregon, PacificSource Community Solutions, and other Oregon Health Plan insurances. Our practice holds a Certificate of Approval with the Oregon Health Authority that enables all of our therapists to work with OHP plans.

    Our Associates (post-graduate) work with all of the insurance plans listed above, as well as Aetna, PacificSource Commercial and Providence.

    In addition to all the plans listed above, Justin Little, LMFT, works with Blue Cross Blue Shield and ComPsych. This means that clients working with Justin and/or a supervisee co-therapist may be able to utilize their insurance.

    We may we able to work with additional plans on a case-by-case basis.

    Please note: While our team will work to do a preliminary check of your benefits before your first appointment, it is always your responsibility to know your benefits and pay any copays/coinsurance fees at the time of each appointment.

  • You can call the phone number on the back of your insurance card. Be sure to ask if you have a deductible that affects your benefits, and if telehealth sessions are different than in-person appointments (when applicable). Please do not hesitate to contact us if you have more questions about using insurance at Forage Wellness. It can be a difficult system to navigate and we are here to support you throughout the process.

  • We accept all major credit cards, many HSA cards (as applicable), cash or check.

  • A Superbill is a document which can be submitted to insurance providers for reimbursement after the session fee is paid at the time of the session. Superbills include:

    Provider’s License type

    Contact details of the provider

    Diagnosis code(s)

    Service procedure code(s)

    Provider’s Tax ID

  • If you are unable to attend a session, please make sure you cancel at least 48 hours in advance. If a cancellation happens within 48 hours of your appointment you will be charged the full cash-pay rate for the session. Unfortunately, insurance carriers do not cover fees for late cancellations. For clients utilizing OHP plan benefits, we will never charge you for a late cancellation, but a pattern of non-attendance to therapy may result in a pause of treatment in order to serve other clients.

  • We ask that you complete your intake packet online at least 48 hours prior to your first appointment. Your first session with your therapist will be an opportunity to learn about confidentiality and consent to treatment, to share more about your current concerns, ask questions, and begin the process of therapy. Our therapists will use the first 3-6 sessions to assess how best to help you and will work with you on what areas to focus first in therapy. Occasionally we work with clients who would be better served by a different resource, and in those cases therapists will work with clients to connect them with appropriate community partners.
